Here at Code Blue for Autism we value your feedback. If you are dissatisfied with any aspect of our service please follow the steps below to help us to improve.  

How to make a complaint

Open the Complaints Form available on this page (click icon to the left). If the document opens in a 'View only' mode, you will have to 'Enable editing'. Please complete the participant section of the form.

When you have completed this section:

  • Send to our team by clicking the mail icon and attaching the completed form to the email

  • or you can send manually to

How we'll respond

Once your form has been recieved by our team we aim to...

  • take immediate action where there appears to be a high risk of harm, neglect or abuse

  • acknowledge complaints within the next business day from receipt

  • call you within two business days of acknowledgement

  • aim to resolve complaints within 21 business days of receipt
